Problema

The patient, being an artist with frequent international tours, informed us from the beginning that he could not attend monthly appointments for orthodontic adjustments. His dental condition included missing teeth in the upper arch and one severely compromised tooth in the lower arch, while the remaining lower teeth could be preserved.

Soluția

The treatment was carried out in multiple stages, starting with multidisciplinary consultations (surgeon, orthodontist, therapist, and prosthodontist) and complete investigations. To adapt the treatment to his lifestyle, self-ligating braces were chosen, as they require less frequent activations. In the upper arch, a fixed All-on-6 FP1 restoration (without artificial gum) was performed, and a sinus lift was carried out on the left side to allow implant placement. In the lower arch, only the irrecoverable tooth was extracted, while the remaining teeth were preserved and included in the orthodontic treatment. After completion of the orthodontic phase, the placement of two lower implants is planned, followed by a decision regarding possible aesthetic improvements (veneers, ceramic crowns, or restorations).

Durata

The orthodontic treatment will last longer than usual, as the patient can attend check-ups only occasionally. Over the course of two years, he returned only twice: the first time after 6 months, and the second time after approximately one year.

Problema

Deteriorated front teeth, decay and pronounced wear, gingival recession and inflammation, signs of periodontal disease, missing teeth and old non-functional restorations, as well as 3 failed old implants.

Soluția

The irreparable teeth and implants were extracted, and the affected areas were thoroughly cleaned. All-on-6 implants were placed in the mandible. In the upper jaw, 6 implants were also inserted, including 3 special implants (one zygomatic and two pterygoid) to ensure stability in areas with reduced bone volume.

• After more than 5 months, the patient returned for 2 days for final impressions and later for another 3 days for try-in and final fixation.

Problema

The patient came to the clinic with the desire to first address the problems in the upper jaw. In this area, she had teeth that had been devitalized for a long time, covered with old crowns that were already chipped, as well as two previously inserted monoblock implants.

Soluția

In the upper jaw, 5 dental implants were placed, positioned at an angle to avoid the need for bone reconstruction in areas where bone was already atrophic. After implant placement, a fixed metal-ceramic prosthesis was made, restoring both chewing function and smile aesthetics. On the left side of the lower arch, a dental implant was inserted, which was later restored with a crown.
Approximately 3 months after the first stage of treatment, the dental bridge on the right side — which the patient had initially chosen to keep against the doctors’ recommendations — failed. Following evaluation, one of the teeth under the bridge had to be extracted. In the remaining spaces, two dental implants were placed, and after the integration period, about 6 months later, two implant-supported crowns were made. In addition, the molar that remained from the old bridge was also covered with a crown to restore the stability and functionality of the dentition.

Conclusion: Sometimes, when doctors’ recommendations are postponed or ignored, unpleasant situations may arise exactly at the moment when you think everything has already been resolved.

Problema

Asymmetrical central teeth: one of the front teeth was broken and shorter than the other.

Soluția

Aesthetic restoration was performed. Caries were also detected in other teeth, which were also restored with photopolymer, but they are not visible in the photo.

Problema

The old and uncomfortable removable denture, which no longer stayed in place without large amounts of adhesive. The patient could no longer taste her food and lived in fear that the denture might fall out while she was laughing or talking. Severe bone atrophy on the upper jaw and several missing teeth on the lower jaw.

Soluția

All-on-X fixed denture using the Quad Zygoma technique (4 zygomatic implants). The X-ray also shows a classic implant in the frontal area, used for better stabilization of the bridge. On the lower jaw, the missing teeth were replaced with traditional implants and zirconium crowns, and the natural teeth were whitened. Two aesthetic composite restorations were also performed.

Problema

Unsatisfactory aesthetics (color and shape of teeth). We do not have the patient’s permission to show the situation before treatment, but the photos after restoration were worth including in our examples.

Soluția

We performed a complete restoration with dental veneers. In fact, the more accurate term would be ceramic restorations, because in the molar area we performed ceramic inlays, onlays, and overlays, several teeth were covered with all-ceramic crowns, and one implant crown was made of zirconium. The shape and color of the veneers were guided by American aesthetic standards.

Problema

Teeth damaged by cavities, discolored; teeth with infection (after poor-quality root canal treatment at another clinic in Moldova).

Soluția

Root canal retreatment and tooth reconstruction; teeth whitening; veneers and all-ceramic crowns on front teeth; three posterior teeth with porcelain-fused-to-metal crowns.
