Rehabilitation on implants without bone graft

Complete dental rehabilitation in Moldova for a dental tourist from Israel, with fixed teeth on implants and no bone grafting

Rehabilitation on implants without bone graft

Problema

The patient presented with severe deterioration of the dentition in both arches. The remaining teeth were affected by extensive decay, major destruction, and loss of bone support, while some areas showed infection and dental mobility. Under these conditions, preserving the existing teeth was no longer a predictable option.

Soluția

A treatment plan focused on complete dental reconstruction was established. In the first stage, the non-restorable teeth were extracted, followed by the placement of dental implants using the All-on-6 technique in both arches, with OSSTEM implants. After the healing period, the provisional restorations were replaced with definitive aesthetic zirconia prostheses, reinforced with a titanium bar, ensuring strength, stability, and a natural-looking smile.

Durata

4 days for the first stage and 1 day for the second stage

From tooth loss to a complete fixed smile

Problema
The patient presented with multiple missing teeth, the remaining teeth being mobile, damaged, accompanied by bad breath, wear, and loss of gingival support. In the lower arch, the situation was unbalanced, with missing teeth and posterior support loss, affecting occlusal stability and chewing function.
Soluția

Both arches were rehabilitated using the All-on-6 technique with traditional dental implants. After surgery, the patient received fixed provisional All-on-X prostheses.
The definitive restorations were made using the Malo Bridge or Thimble Bridge concept, which involves a special bar supporting individual zirconia crowns for each tooth. The major advantage of this type of restoration is reparability — any crown can be replaced if necessary without remaking the entire bridge.

Durata

5 days – fixed provisional teeth on implants (plus 2 days for check-ups)
after 6 months (implant integration period) – fixation of the definitive prostheses

From compromised teeth and old restorations to fixed teeth on All-on-6 implants

Problema

The patient had a severely compromised dentition due to previous treatments, unaesthetic metal and metal-ceramic crowns, gingival inflammation, and weakened or destroyed teeth. The existing restorations no longer provided stability, and the sources of infection had to be eliminated.

Soluția

Nine compromised teeth were extracted and 6 implants were placed in each arch. On the left side, one of the implants was angled to avoid sinus lifting. Immediately after the intervention, the patient received fixed provisional teeth using the All-on-6 concept. These are not visible on X-rays because they are made of provisional material, which is less radiopaque than metal or zirconia. In the final version, the restoration will include 12 teeth on each arch.

Durata

7 days – from consultation to the final check-up;
5 days – from implant placement to long-term provisionals;
24 hours – first set of fixed teeth.

Smile reconstructed with All-on-6 and hidden access holes

Problema

A patient from France chose to undergo treatment in Chișinău, presenting with a severely compromised dentition and the need to replace an old partial removable denture with a complete fixed rehabilitation on implants.

Soluția

After the extraction of the compromised teeth, 6 OSSTEM dental implants were placed in each arch, and the patient received fixed provisional All-on-X restorations. A particular feature of this case was the use of special dynamic abutment screws on 3 implants, to modify the direction of the screw access channel. This allowed the closure holes to be positioned occlusally or lingually/palatally, avoiding their visibility when the patient smiles. In the end, definitive fixed zirconia restorations reinforced with a titanium bar were delivered.

Durata

At the time of treatment, in 2024, the final prosthetic stage lasted 10 days. Today, the same protocol can be completed in approximately 7 days.

From compromised teeth to fixed teeth on implants

Problema

The patient presented with teeth previously treated endodontically, affected by infections, cavities, fractures, and old unaesthetic restorations. As a patient from Italy, coming for dental tourism, she chose our clinic also for the careful organization, linguistic support, and logistical assistance during her stay in Moldova.

Soluția

After the extraction of the compromised teeth, dental implants were placed in both arches. In the upper jaw, due to the reduced stability of the two distal implants, the provisional restoration was initially fixed on 4 implants and included only 10 teeth. In the lower jaw, the bone allowed the uniform placement of 6 implants. In the end, two fixed zirconia prostheses were delivered, each with 12 teeth, without cantilever and without a titanium bar.

Durata

The first stage lasted 7 days. The final prosthetic stage lasted 12 days, including the time required to install 2 MUAs on the distal implants and for the gum to heal after a small incision.

Fixed teeth without bone graft in cases of atrophic bone

Before and after – provisional All on X prosthetic restoration

Fixed teeth without bone graft in cases of atrophic bone

Problema

The patient presented with a severely compromised dentition. The old metal-ceramic restorations had lost their stability, and some of the teeth could no longer be saved. The situation was further complicated by bone volume loss in the posterior area of the upper jaw, which made the insertion of conventional implants difficult without special surgical solutions.

Soluția

The compromised teeth and old crowns were removed, and dental implants were subsequently placed to support a fixed provisional restoration on both arches. In the upper jaw, where bone volume was reduced, pterygoid implants were used – a modern solution that allows stabilization of the prosthesis without the need for complex bone grafting procedures.

Durata

4 days after the intervention – which is why the sutures can still be observed in the photo under the lower provisional bridge.

Fixed teeth on implants with the All-on-6 technique and pterygoid implants

Problema

The patient came to Punto Bianco Clinic with multiple mobile and severely damaged teeth, bad breath, serious chewing difficulties, and a smile that was significantly affected aesthetically.

Soluția

This clinical case was performed using an extended All-on-4 concept, by adding two additional posterior implants: pterygoid implants (Brand JDI) in the maxilla and distal implants parallel to the mandible, resulting in a fixed All-on-6 rehabilitation.

The patient chose to undergo the intervention under General Anesthesia for maximum comfort.

After the integration period of the implants, the final restoration was made of zirconia reinforced with a titanium bar, offering an optimal combination of strength, stability, and natural aesthetics.

Durata

As the patient is a sailor, his vacations are less frequent but longer, so the treatment was planned according to his schedule. The first stage lasted 21 days in Moldova, and the final prosthetic stage took place almost one year later and lasted nearly 2 weeks.

Complete oral rehabilitation for a patient from the diaspora

Complete oral rehabilitation for a patient from the diaspora

Problema

The patient from the Moldova, living in Denmark, had missing, fractured, and periodontally affected teeth. The case required complex full oral rehabilitation, carefully planned. For patients coming from abroad, precise treatment scheduling is essential to avoid delays, postponements, or missed flights.

Soluția

Compromised teeth were extracted and 6 OSSTEM implants were placed in each arch. In the same stage, fixed provisional ALL on X prostheses were delivered. Later, the final zirconia restorations with ceramic gingiva provided a stable, aesthetic, and easy-to-maintain result. In the upper jaw, the prosthesis was internally reinforced with a hidden metal bar.

Durata

The first stage of treatment lasted 11 days, and the fixed provisional teeth were delivered in just 2 days. After 7 months, the patient returned for the final stage, which lasted 9 days. Proper planning allowed the patient to strictly follow the planned stay period in Moldova.

When doctors’ recommendations are postponed or ignored

Example of staged treatment with dental implants

When doctors’ recommendations are postponed or ignored

Problema

The patient came to the clinic with the desire to first address the problems in the upper jaw. In this area, she had teeth that had been devitalized for a long time, covered with old crowns that were already chipped, as well as two previously inserted monoblock implants.

Soluția

In the upper jaw, 5 dental implants were placed, positioned at an angle to avoid the need for bone reconstruction in areas where bone was already atrophic. After implant placement, a fixed metal-ceramic prosthesis was made, restoring both chewing function and smile aesthetics. On the left side of the lower arch, a dental implant was inserted, which was later restored with a crown.
Approximately 3 months after the first stage of treatment, the dental bridge on the right side — which the patient had initially chosen to keep against the doctors’ recommendations — failed. Following evaluation, one of the teeth under the bridge had to be extracted. In the remaining spaces, two dental implants were placed, and after the integration period, about 6 months later, two implant-supported crowns were made. In addition, the molar that remained from the old bridge was also covered with a crown to restore the stability and functionality of the dentition.

Conclusion: Sometimes, when doctors’ recommendations are postponed or ignored, unpleasant situations may arise exactly at the moment when you think everything has already been resolved.

Durata

2 years (see reasons above)
