Problema

The patient, originally from Romania and settled in Canada, chose to come to Moldova following the recommendation of a couple who were satisfied with the results obtained at Punto Bianco. At the time of presentation, her dental condition was complex: she was wearing an old metal-ceramic restoration that was deteriorated and unaesthetic, while the remaining teeth were affected by periodontal disease and mobility. In addition, severe bone atrophy was identified in the upper jaw, significantly complicating treatment options.

Soluția

In this complex case, the treatment was performed under general anesthesia, using a combination of conventional, pterygoid, zygomatic, and one short implant in the lower left area, adapted to the available bone. Some patients may wonder why implants were placed in areas without visible teeth. Their role is to stabilize the entire structure—especially the zygomatic implants—and to provide additional support for the definitive restoration, ensuring long-term strength and durability.

Durata

The first stage lasted 9 days, as it included not only the surgical intervention but also thorough preparation: blood tests, imaging investigations, consultation with the anesthesiologist, and detailed planning of the special implants. After the integration period, the second stage lasted 11 days—slightly longer than usual—to allow the final restoration to be reinforced with a supporting bar and to ensure optimal long-term resistance.

Problema

The patient, being an artist with frequent international tours, informed us from the beginning that he could not attend monthly appointments for orthodontic adjustments. His dental condition included missing teeth in the upper arch and one severely compromised tooth in the lower arch, while the remaining lower teeth could be preserved.

Soluția

The treatment was carried out in multiple stages, starting with multidisciplinary consultations (surgeon, orthodontist, therapist, and prosthodontist) and complete investigations. To adapt the treatment to his lifestyle, self-ligating braces were chosen, as they require less frequent activations. In the upper arch, a fixed All-on-6 FP1 restoration (without artificial gum) was performed, and a sinus lift was carried out on the left side to allow implant placement. In the lower arch, only the irrecoverable tooth was extracted, while the remaining teeth were preserved and included in the orthodontic treatment. After completion of the orthodontic phase, the placement of two lower implants is planned, followed by a decision regarding possible aesthetic improvements (veneers, ceramic crowns, or restorations).

Durata

The orthodontic treatment will last longer than usual, as the patient can attend check-ups only occasionally. Over the course of two years, he returned only twice: the first time after 6 months, and the second time after approximately one year.

Problema

Deteriorated front teeth, decay and pronounced wear, gingival recession and inflammation, signs of periodontal disease, missing teeth and old non-functional restorations, as well as 3 failed old implants.

Soluția

The irreparable teeth and implants were extracted, and the affected areas were thoroughly cleaned. All-on-6 implants were placed in the mandible. In the upper jaw, 6 implants were also inserted, including 3 special implants (one zygomatic and two pterygoid) to ensure stability in areas with reduced bone volume.

Durata

• 7 days – first stage = fixed provisional teeth
• After more than 5 months, the patient returned for 2 days for final impressions and later for another 3 days for try-in and final fixation.

Problema

Severely affected dental condition, with destroyed and decayed teeth covered by old, non-functional restorations, gingival inflammation and active infections, missing teeth and extended edentulous spaces, chewing difficulties, and constant discomfort.

Soluția

Irreparable teeth were extracted and old restorations removed. All-on-6 implants were placed in the upper jaw and All-on-5 in the mandible. The intervention was performed under controlled conditions, under general anesthesia. Fixed provisional restorations were applied, followed later by definitive zirconia restorations supported by implants. The final works were stabilized through abutments and precise occlusal adjustments.

Durata

– 3 days for consultations, investigations, blood tests, and planning
– 4.5 hours under general anesthesia: removal of old restorations, extractions, implant placement in both jaws, and impressions
– 10 days for the entire first stage, although currently these procedures can be completed in a much shorter timeframe

Problema

The patient from France presented with an extremely complex case: failed monoblock implants connected prosthetically to natural teeth, bone loss, and old compromised restorations. Although attempts were made to save some teeth, they could no longer be preserved due to extensive destruction, posts, and aggressive previous preparations.

Soluția

Under general anesthesia, difficult extractions of the failed implants and compromised teeth were performed, followed by the placement of traditional and special dental implants. In the final stage, the patient benefited from a premium fixed rehabilitation: an aesthetic zirconia restoration with individual crowns on a bar.

Durata

Considering the complexity of the case, the patient reserved 14 days for the first stage and another 14 days for the final stage. As a clinic organized for dental tourism, we planned the treatment so that the stay in Moldova was efficient yet comfortable, with time for relaxation, excursions, and shopping.

Problema

The patient, living in Italy, wore old removable dentures that were uncomfortable and unstable, affecting chewing, comfort, and smile aesthetics. The case required complete rehabilitation on implants.

Soluția

Dental implants were placed in both arches, with immediate loading through fixed provisional All-on-X restorations. After osseointegration, the provisional restorations were replaced with definitive fixed prostheses, providing stability, comfort, and a natural appearance. The case is well documented with before-and-after photographs, including images of the old removable dentures.

Durata

The first stage lasted 12 days, while today a similar protocol can be completed in 4–7 days. The final prosthetic stage lasted 9 days, and currently, thanks to the fully digital protocol, it can be finalized in 6–7 days.

The patient came from Belgium and had a very strong fear of dental treatment, which is why she requested general anesthesia. She had a severely compromised dentition in the upper jaw and significant bone loss.

Soluția

The compromised teeth were extracted. To ensure a solid foundation, bone grafting procedures were performed in the anterior area and sinus lifting was carried out, followed by implant placement according to the All-on-6 technique in the upper jaw and All-on-4 in the lower jaw. Immediately after the intervention, the patient received fixed provisional teeth. After the healing period, these restorations were replaced with definitive fixed zirconia prostheses reinforced with a titanium bar.

Problema

The patient presented with a generalized compromised dental condition, characterized by dental mobility, pronounced gingival inflammation, and visible gum recession. The anterior teeth were migrated and misaligned, and the presence of deposits and discoloration indicated difficult oral hygiene in the context of periodontal disease. The diagnosis revealed advanced chronic periodontitis.

Soluția

Considering the complexity of the case, a complete rehabilitation using the All-on-6 technique was chosen for the upper jaw. OSSTEM dental implants were placed according to the All-on-6 protocol, providing a stable foundation for the fixed restoration, and a provisional All-on-X prosthesis was delivered. Through this complex treatment stage, the patient achieved full stabilization of the dentition and elimination of active periodontal problems.

Problema

The patient, originally from Ukraine and living in Italy, presented with a complete removable denture in the upper jaw and old compromised restorations in the mandible, associated with infections and dental mobility. In the upper jaw, the bone was severely atrophic, so fixed rehabilitation would have been possible only with special implants.

Soluția

For financial reasons, the patient chose to treat only the mandible. The intervention was performed under general anesthesia, and after implant placement, the patient received fixed provisional teeth within 24 hours. The upper jaw remained untreated for the moment, although it would have required zygomatic, pterygoid, and short implants in the anterior area.

Durata

The first stage lasted 8 days, including investigations, consultations, and anesthesiological evaluation. After implant placement, the fixed provisional teeth were delivered within 24 hours.
